Ten Step Housing Stability Plan:
Households Currently in Danger
1. Always First Ensure Immediate Safety
Households Currently on the Street
2. Address other Immediate Needs such as shelter or food
Households Currently Experiencing Homelessness in Shelter or on the Street
3. Problem Solving/Resource Exploration
4. Housing Plans and Preferences
5. Creating An Action Plan
6. Preparing for Transitions into Housing
Households that have now Obtained Housing Stability after experiencing homelessness
7. Goal Setting
8. Working Through Trauma
9. Give Back by Volunteering and Helping Others
10. Elevate Voices of those experiencing homelessness and ongoing advocacy
Resources Based on HUD Homeless Definitions:
Someone that is facing imminent risk of becoming homeless within 14 days, has nowhere else to go and lacks the resources and support to not become homeless.
Someone that is living in emergency shelter (agency contracts with DSS and require DSS application to access) or a winter shelter program.
Someone that is living unsheltered (tent in the woods, train stations, behind buildings, living in their vehicle, etc.)
Someone that is actively fleeing violence with no where to go and no resources not to become homeless.
Chronic Homelessness means that someone has been living on the street or in shelter for longer than one year and has a permanent disability.
Emergency Housing Vouchers
Emergency Housing Vouchers (EHV) have been issued to approximately 450 homeless households throughout Suffolk County and Nassau County. This voucher program was funded by HUD as part of COVID-19 emergency response to exit households out of shelters, and homelessness at large, where people are at higher risk of exposure to COVID-19.
