Coordinated Entry Assessment

The local assessment tool focuses on and barriers to exiting homelessness as a guiding tool to determine who will be prioritized for permanent housing resources first. 

  • Meeting the person where they are, in a safe/preferred location of their choosing (person-centered)

  • Completing an intake form with information about housing and service goals (housing-focused)

  • Completing a housing and services preference form (person-centered)

  • Completing a formal housing plan (critical time intervention)

  • Completing a trauma recovery exercise (trauma-informed care)

  • Safety planning (DV households)

  • Completing any necessary consent forms for required medical documentation related to obtaining housing (service needs)

  • Exploring and reviewing all available and viable housing options (housing-focused)

  • Completing housing applications (housing-focused)

  • Connecting to community supports (support systems)

Chronically homeless households either on the street or in shelter are regularly engaged by LICH Coordinated Entry staff until permanently housed  (assertive housing-focused engagement and transition case management/critical time intervention).

LICH staff are currently working with single adults living on the street or in shelter that have not been able to exit homelessness for several years.
