Please note: Long Island Coalition for the Homeless has two types of street outreach teams.
Chronically Unhoused Street Outreach Criteria: Verification needed of a client living unsheltered for a year or longer. To complete a referral form for your client: https://www.lihomeless.org/ces-referral
Safe Options Support Team Outreach Criteria: Persons in need of intensive intervention due to multiple high barriers to accessing housing, who are experiencing homelessness in shelter or living unsheltered
To complete a referral for your client: https://www.lihomeless.org/sos
LICH does not accept direct referral for case management services for clients living in shelter. This is because through the Homeless Management Information System, LICH can see who must be prioritized and assigned based on their length of time homeless.

What is Street Outreach?
Street outreach teams go out into the community to connect with people living unsheltered and try to connect them to services. Some teams can provide essential items, such as food, clothes, and/or toiletries. Others provide housing-focused case management, substance use or mental health support.
